You must

– hold a degree or qualification in Advanced Practice
– be a V300 Prescriber

Skill Set / Experience
To be eligible for this role you must:

– be an independent prescriber
– be able to confidently see, treat and discharge patients
– have experience of working with SystmOne
– have a smartcard

Community Nurse ( Band 5)

Hallam Medical have work available for Band 5 Community Nurses across South Oxfordshire

We are looking for band 5 Community Nurses who can work from any of the following locations.
•             Henley
•             Goring
•             Nettlebed
•             Wallingford
•             Watlington
•             Wheatley
•             Berinsfield

 

About the Role
As part of this role you will be providing care to housebound patients for the following:

Wound care, including:

•             Leg ulcer care
•             Chronic wounds
•             Pressure ulcers
•             Lacerations
•             Post-surgical wounds
 

Non-oral medication administration, including:

•             Insulin, OD and if required, BD regimes, including blood glucose and ketone monitoring
•             Low molecular weight heparin
•             Denosumab
•             B12
•             Filgrastim
•             IV medication via PICC lines, such as antibiotics, flushes, bung and dressing changes **only if a specific signed competency/experience
•             Eyedrops – may be delegated to formal carers, with training
•             Eardrops – may be delegated to formal carers, with training
•             Application of Transdermal patches

Continence care
•             Female, male and supra-pubic urinary catheters
•             Continence assessments, treatments, reviews and product provision if required following assessment
•             Bowel care, including constipation, and routine bowel management including anal/rectal irrigation (which would likely be delegated to formal carer with training)

Phlebotomy for housebound patients

General Documentation Skills

Basic Assessment Skills for pressure damage prevention, activities of daily living, mobility etc.

This opportunity is ongoing and offers 37.5 hours a week, with a choice of shift patterns; Monday to Friday - either 08:30 to 16:30 or 11:00 to 19:00

Educational Requirements

To apply for this role you must be:

•             An NMC Registered Nurse with experience of working in a community setting. ( Band 5)

As this role is based in the community it is essential you are a car driver and have access to a vehicle.

If you haven’t been trained in the any of the above or are a little rusty, please apply as the service offers both training and up-skilling for the right candidates.
